For wisdom teeth that are severely impacted, we may recommend to perform the procedure under intravenous sedation or general anesthesia due to the difficulty of the extractions. In some cases wisdom teeth can be removed under local anesthesia, when the degree of impaction is not severe, the position of the impacted tooth is favorable, or the tooth is erupted into the oral cavity.

Intravenous sedation is also known as conscious sedation or “twilight sleep”. This allows the surgeon to perform the procedure with minimal discomfort to the patient. Intravenous Sedation is an anesthetic requiring that the patients not have anything to eat or drink for eight hours prior to treatment and to be accompanied by a responsible adult to safely transport them home.

Why Remove Wisdom Teeth?

There are four main effects of wisdom teeth impaction requiring tooth extraction to preserve patient well-being. Wisdom teeth need to be extracted in cases of:

Cavities
Wisdom tooth cavities result from tooth decay in the region of the wisdom tooth and affect neighbouring teeth. Food becomes trapped in the area of the irregular-developed wisdom tooth because it is difficult to clean. Excess oral bacteria colonise where plaque sticks to the trapped food, leading to the development of gum inflammation and infection. Extracting the wisdom tooth can prevent the spread of tooth decay to other dental structures.

Cysts
Wisdom tooth cysts form from fluid and gas build-up in the infected bone around the impacted wisdom tooth. Cysts may grow and vary in size requiring enucleation and wisdom tooth extraction, so as to remove infection and prevent its spread in the body.

Gum Disease
Wisdom tooth gum disease results from an inability to properly clean the impacted wisdom tooth. Plaque and excess bacteria colonise the food debris trapped in the region of the wisdom tooth. If left untreated gum disease can then develop. To preserve oral and overall health and prevent further bone loss the wisdom tooth is extracted.

Pericoronitis
Wisdom tooth pericoronitis is caused when the gum tissue or operculum overlapping the wisdom tooth becomes infected by colonising bacteria and plaque. The infection often re-occurs and results in the need for wisdom tooth extraction.

Wisdom Teeth Development and Impact

Wisdom teeth have undergone an evolutionary change in their development due to modern civilisation. In earlier centuries before the use of cutlery to eat when humans needed to be able to chew raw or semi-raw meats, roots and other foods, wisdom teeth served a useful and powerful purpose

Over time modern living and diet have changed the way humans evolve physically, including smaller jaw development and impacted wisdom teeth. Some people still develop wisdom teeth and others do not. Wisdom teeth may erupt causing dental disorders and impacted teeth, requiring their extraction.

When Do Wisdom Teeth Issues Arise

Between the ages of 18 and 25 wisdom teeth begin to develop. Wisdom teeth form in the back corners of the upper and lower jaw and may consist of one to four third molars. Some people develop no wisdom teeth while others only have one.

Wisdom teeth can become impacted when the jaw is too small to accommodate their growth. This requires dental treatment otherwise impaction may contribute to the wisdom teeth becoming infected.

Wisdom tooth infection occurs when the impacted wisdom tooth causes gum or operculum swelling and inflammation. Bacteria may colonise around the inflamed wisdom tooth leading to infection. If not properly cleaned and removed, trapped food may also cause plaque and bacterial build-up leading to infection.

Tooth Extraction Pain & Aftercare

Having your tooth extracted can be a little bit uncomfortable for a couple of days afterwards, but we can manage that with pain killers and it usually settles down quite quickly. Your healing time will largely depend on what kind of anaesthetic you received. If it was a local anaesthetic, it will be a lot less than if you were given IV sedation.

You will experience some bleeding, bruising and swelling in the first few days after surgery so be sure to follow your dentist’s recommendations closely when it comes to pain management, smoking, cleaning, eating and drinking.

The impaction can lead to decay and resorption of healthy teeth. On occasion, if wisdom teeth are not monitored properly, their growth can shift parallel to the jawline. They can also shift backward and eventually interfere with the opening and closing of your jaw.
 
All impacted wisdom teeth do not need to be removed. If the impacted wisdom tooth is causing problems, then it will probably have to be removed, but not otherwise. An impacted wisdom tooth takes place when your wisdom teeth grow at an awkward angle, or if there is not enough room for them.
Impacted wisdom teeth can result in pain, damage to other teeth and other dental problems. In some cases, impacted wisdom teeth may cause no apparent or immediate problems. But because they're hard to clean, they may be more vulnerable to tooth decay and gum disease than other teeth are.
Impacted wisdom teeth are more difficult to remove, leave you at greater risk for complications from surgery, and can permanently damage bones and other teeth. Also, the longer wisdom teeth pain persists, the more likely it is that an infection will result from bacteria entering open tissue.

A common misconception is that it may be necessary to “break the jaw” to remove difficult wisdom teeth. However, this is never the case.

It takes up to six weeks for your mouth to completely heal. You probably won't be able to open your mouth normally for about a week, so you'll need to eat soft foods. After surgery, you may experience some pain, bleeding, and swelling.
